Resumo

INTRODUÇÃO: Projéteis de alta velocidade (PAV; > 600 m/s) geram canais permanentes e cavidades temporárias de até 14 vezes o calibre, com pressões de 1-3 MPa, criando padrões de lesão específicos[cite: 3, 17, 30]. Reconhecer esses padrões é crucial para cirurgia urgente, interpretação radiológica e reconstrução forense[cite: 4, 18, 31]. MATERIAIS E MÉTODOS: Uma revisão integrativa, guiada pelo PRISMA 2020, buscou em PubMed, Embase, Scopus/Web of Science e Cochrane (1984-2025)[cite: 5, 19]. Após triagem e avaliação de textos completos, 18 estudos foram incluídos[cite: 6, 20]. DISCUSSÃO: A transferência de energia aumenta quadraticamente com a velocidade[cite: 7, 21]. Acima de 600 m/s, a cavidade temporária pode atingir um raio ≥ 5 cm em fígado bovino[cite: 8, 22]. Órgãos com baixo módulo de elasticidade (fígado, baço, rim) apresentam lacerações em "estrela"[cite: 9, 23], enquanto o pulmão forma canais tubulares[cite: 9, 23]. Achados típicos incluem orifícios de entrada circulares com colar de escoriação e saídas irregulares evertidas com razão diâmetro EX/ENR > 2[cite: 11, 25]. A tomografia computadorizada helicoidal com angiografia (TCA) é o padrão-ouro[cite: 12, 26]. CONCLUSÃO: A morfologia das lesões por PAV decorre da interação não linear entre balística, viscoelasticidade tecidual e fragmentação[cite: 13, 27].

Abstract

INTRODUCTION: High-velocity projectiles (HVP; > 600 m/s) generate permanent crush tracks and temporary cavities up to 14 × caliber, with 1–3 MPa pressures, producing specific wound patterns. Recognizing these signatures is essential for urgent surgical conduct, radiological interpretation, and ballistic forensics. MATERIALS AND METHODS: An integrative review, guided by PRISMA 2020, searched PubMed, Embase, Scopus/Web of Science, and Cochrane (1984-2025). After screening and full-text appraisal, 18 studies were included. DISCUSSION: Energy transfer increases quadratically with velocity. Above 600 m/s, the temporary cavity can reach a ≥ 5 cm radius in bovine liver. Low-elastic-modulus organs (liver, spleen, kidney) exhibit star-burst lacerations, while lung forms tubular channels. Typical findings include circular entrance holes with abrasion collars and irregular everted exits with an EX/ENR ratio > 2. Helical CT angiography (CTA) is the gold standard. CONCLUSION: HVP wound morphology results from the non-linear interplay between ballistics, tissue viscoelasticity, and fragmentation.
