THE IMPORTANCE OF HISTOPATHOLOGICAL TECHNIQUES IN DEFINING THE CAUSE OF DEATH ASSOCIATED WITH HYPERTROPHIC CARDIOMYOPATHY: CASE REPORT
Resumo
Em alguns casos, os achados necroscópicos resultantes de um crime por si só não são capazes de justificar a morte de um individuo. Nesses episódios, além de realizar uma investigação criteriosa, o legista precisa recorrer a outros mecanismos para tentar identificar a causa mortis. Dessa perspectiva, a histopatologia forense se torna uma ferramenta importante, pois, muitas vezes, os achados teciduais auxiliam na definição de hipóteses. Nesse relato de caso, apresenta-se a necropsia de um homem de 42 anos que foi recebido no IML com a história de óbito após soco na região orbitária direita. Ao exame necroscópico foi observada pequena sufusão hemorrágica subcutânea periorbitária superior à direita e hematoma subgaleal parietoccipital à esquerda (compatível com queda ao solo), contudo sem presença de fraturas cranianas ou traumas encefálicos. Além disso, foi evidenciado que o coração apresentava grande volume, com hipertrofia do ventrículo esquerdo. Este órgão foi retirado e enviado a anatomia patológica, que por meio de cortes histológicos observou a presença de núcleos hipercromáticos nos cardiomiocitos, múltiplas áreas de desarranjo arquitetural das fibras cardíacas e fibrose intersticial, bem como hipertrofia acentuada, características da cardiomiopatia hipertrófica, doença genética associada a arritmias e a morte súbita. Essas informações foram essenciais para definir que o mecanismo de morte provavelmente estava associado ao quadro cardíaco, afastando a possibilidade de morte por traumatismo contundente.

Abstract
In some cases, postmortem findings resulting from a crime alone are not sufficient to justify an individual's death. In these situations, in addition to conducting a thorough investigation, the medical examiner needs to resort to other mechanisms to try to identify the cause of death. From this perspective, forensic histopathology becomes an important tool, as tissue findings often assist in defining hypotheses. In this case report, the autopsy of a 42-year-old man received at the IML with a history of death following a punch to the right orbital region is presented. The postmortem examination revealed a small subcutaneous hemorrhagic suffusion superior to the right periorbital area and a parieto-occipital subgaleal hematoma on the left (consistent with a fall to the ground), however, without evidence of cranial fractures or brain traumas. Additionally, it was noted that the heart was enlarged, with hypertrophy of the left ventricle. This organ was removed and sent for pathological examination, which, through histological sections, observed the presence of hyperchromatic nuclei in the myocardial cells, multiple areas of architectural disarray of the cardiac fibers, interstitial fibrosis, as well as marked hypertrophy, characteristic of hypertrophic cardiomyopathy, a genetic disease associated with arrhythmias and sudden death.These findings were essential to conclude that the likely mechanism of death was associated with the cardiac condition, ruling out the possibility of death by blunt trauma.
