ATYPICAL HANGING – A CASE REPORT OF DEATH BY CERVICAL CONSTRICTION AND THE ROLE OF FORENSIC MEDICINE IN CLARIFYING A POSSIBLE STAGED SUICIDE SCENE
Resumo
INTRODUÇÃO: O enforcamento é uma forma de asfixia por constrição cervical, onde a força que aciona o laço é o peso da própria vítima. A causa jurídica mais comum é o suicídio, sendo raro o enforcamento homicida. No entanto, o médico legista deve estar atento ao enforcamento atípico, pois a cena pode ser manipulada para simular um suicídio. O nó cervical, quando localizado na região anterior, caracteriza-se como atípico. Este estudo descreve as particularidades do exame necroscópico de um cadáver com morte por enforcamento atípico. MÉTODO: Relato de caso de um indivíduo masculino de 69 anos, que foi submetido a necropsia no Instituto Médico Legal de Minas Gerais. O exame externo revelou um sulco cervical único, oblíquo e interrompido na região cervical anterior. Informações da perícia de local auxiliaram na análise dos achados necroscópicos. A suspensão foi incompleta, com os pés tocando o solo. RESULTADOS: O exame interno revelou sufusões hemorrágicas no sulco cervical, músculos profundos, e na musculatura paravertebral, com fratura do processo odontoide e o osso hioide íntegro. DISCUSSÃO: O exame detalhado e a colaboração com a perícia criminal são essenciais para diferenciar suicídios simulados de homicídios. Exames complementares, como a anatomia patológica, são fundamentais para elucidar a causa da morte. CONCLUSÃO: A perícia de local é crucial no esclarecimento de mortes por enforcamento atípico.

Abstract
INTRODUCTION: Hanging is a form of asphyxia by cervical constriction, where the force that activates the noose is the weight of the victim's own body. The most common legal cause is suicide, with homicidal hanging being rare. However, the forensic pathologist must be alert to atypical hanging, as the scene may be manipulated to simulate a suicide. The cervical knot, when located in the anterior region, is characterized as atypical. This study describes the particularities of the necroscopic examination of a cadaver with death due to atypical hanging. METHOD: Case report of a 69-year-old male individual who underwent necropsy at the Legal Medical Institute of Minas Gerais. The external examination revealed a single, oblique cervical groove, interrupted in the anterior cervical region. Information from the crime scene investigation assisted in analyzing the necroscopic findings. The suspension was incomplete, with the feet touching the ground. RESULTS: The internal examination revealed hemorrhagic suffusions along the cervical groove, in deep muscles, and in the paravertebral musculature, with a fracture of the odontoid process and an intact hyoid bone. DISCUSSION: Detailed examination and collaboration with the criminal investigation are essential to differentiate simulated suicides from homicides. Complementary examinations, such as pathological anatomy, are crucial for elucidating the cause of death. CONCLUSION: Crime scene investigation is crucial in clarifying deaths from atypical hanging.
